Jack Perkins

Mr. Jack Timothy Perkins, age 64, of Dover, passed away, Tuesday, October 9, 2012 at Patient's Choice Medical Center, Erin, TN. A funeral service will be held at 7:00 pm Friday at Anglin Funeral Home with Bro. Peter Flournoy to officiate. Visitation will be from 5 - 7 pm Friday at Anglin Funeral Home. His remains are to be cremated. He was born December 31, 1947 in Salem, Ohio, son of the late Leonard and Delores Halderman Perkins. Mr. Perkins was a US Navy Vietnam Veteran and retired Civil Service. He is survived by his wife, Judith M. Duriga Perkins, children, Richard Perkins (Myla), Jacksonville, FL, Letisha Thomas (Matt), Stewart, TN, Delores Stokes (Jared), Ft. Hood, TX, brother, Robert Perkins (Arlene), Colorado Springs, CO, and eight grandchildren. In addition to his parents he is preceded in death by his brother, Edward Perkins. In lieu of flowers, the family requests donations be made to a charity of your choice.
